Application Instructions

A COMPLETED APPLICATION PACKET MUST INCLUDE:

I) Application for post-baccalaureate status in the Graduate School, if not currently enrolled in a graduate program  (domestic applicants: $40.00, international students: $55.00).

Domestic applicants who are not UK faculty will need to provide a photocopy of an official transcript from the institution in which you received your undergraduate degree (Fax:  859 323-1928).

International applicants who are not UK faculty will need to provide a photocopy of an official transcript from the institution in which you received your undergraduate degree (or equivalent), official TOEFL or IELTS scores, and visa status  (Fax:  859 323-1928).

II) Complete the Graduate certificate application form (copy attached).

III) Photocopies of official transcripts from the institution in which you received your undergraduate, graduate, and/or professional degrees, and/or current transcript from graduate or professional program.

IV) A Letter of Recommendation from your mentor, program director or academic department chair. This letter should reflect your career potential in Clinical and Translational Science as well as address the amount of time you will have available to participate in the training program.

V) Curriculum Vitae including any presentations, publications (abstracts, manuscripts, book chapters, etc.).

Tuition Note:  Enrollment in graduate level courses generates tuition expenses, as specified at the UK registrar’s website.  Full-time faculty and staff at UK chould check with their department administrators for information on employee tuition benefits and for instructions on accessing those benefits. UK postdoctoral appointees and graduate students and applicants not affiliated with UK may apply for tuition scholarships.
